Doll Hospital Question

I have decuded that my "bubble eye" Kirsten needs a new set of eyes...
I know I have to include a note asking them to only replace her eyes and to leave her neck strings, etc...
BUT
Is there anything else I have to include? Since they are flipping the bill to replace her eyes, do I still need to fill out one of those doll admission forms from the website??
Any advice would be greatly appreciated!!

I dropped a doll off at the AG Hospital this week. The person in CS did not fill out all of the admissions info. She just wrote in BIG BLACK letter: REPLACE EYES ONLY. DO NOT REPLACE HEAD OR ANYTHING ELSE I then filled out my shipping and billing info and added my phone number. So far I haven't heard anything.

When I mailed in all 3 of my dolls that needed new eyes last summer CS told me that I had to fill out the admissions form for a new head and include a note saying that I wanted only the eyes replaced and the original strings attached. They replaced my two AGT 3 eyes and gave Kirsten a much needed head and limbs replacement. I didn't specify on Kirsten because I really wanted a new head for her. Her hair was really ratty.

BUBBLE EYE!!?? WHAT IS THAT!?
Is that something the dolls can get over time? or is it something that they just accidentally come with?
It sounds "painful!" lol.

I would ask if you can keep the old eyes too. She is a PM after all.
